The Youth Empowerment Services Waiver is a 1915(c) or the YES Waiver is a Medicaid program that is provided for children and youth who have serious mental, emotional, and behavioral health issues. The services covered by this program are focused on strengthening the support system of the child by helping the family prevent out-of-home placement, promote lifelong independence and self-defined success.

At Reassure Health LLC, we meet with you to create a Wraparound Plan that is made with you and your child in mind. We are committed to providing a tailored services for your family. The following are requirements for individuals who wish to avail of the YES waiver:

  • Be 3 through 18 years old.
  • Have serious mental, emotional and behavioral difficulties.
  • Have a qualifying mental health diagnosis.
  • Be at risk of being placed outside of their home due to their mental health needs.
  • Meet the criteria to be in a psychiatric hospital.
  • Be eligible for Medicaid
  • Currently live in a home setting with a legal guardian or on their own if they are legally emancipated.

  • Animal-assisted therapy
  • Art therapy
  • Music therapy
  • Recreational therapy
  • Nutritional counseling
Community Living Support
Through proven strategies, we can help families adjust to the challenges they may face due to their loved ones’ mental health conditions.

Employee Assistance and Supported Employment
We can help youth find their potentials and the job that is fit for them.
Paraprofessional Services
Giving you the support that you need during stressful situations through mentoring and coaching.
Respite Services
Providing family caregivers the chance to take the break that they need from their responsibilities.
Non-medical Transportation
We have vehicles available to take you to your appointments with us when no other option is available.
Supportive Family-Based Alternatives
Giving family members that support that they need to help them have healthier interactions while the youth lives in a therapeutic home.
Adaptive Aids and Supports
Helping youth avoid out-of-home placement by providing supplementary medical assistance to support the YES services that the client is currently having.
Minor Home Modifications
Making your home more conducive for your loved ones’ well-being.
Transition Services
Help with the costs associated with youth moving into their own home.
